These forms may be printed out by parents as needed and given to the coach on the day the situation occurs.

This form may be printed out by parents as needed and given to the coach up to or on the day the situation occurs.

This form needs to be signed by student and parent and returned to the coach. This will be kept on file with the Athletic Department.

Explains the Athletic Permission Form and provides information regarding Student Accident Insurance and athletic equipment responsibilities. Page Two of the Parent Letter must be completed by the parent and returned to the coach to be kept on file in Team Medical Kit in case of emergency and on file with the Athletic Department.

This packet should be completed by parents, students and medical professional and on file with the school before the first day of practice or at the time of the free physical dates held in the school. Follow the instructions regarding completing this form. If your student participates in more than one sport during the school year, retain the Section 7 to use at that time.
